If you're applying for a Green Card (also called permanent residency) in the United States, you’ll need to get a special medical check-up called an Immigration Medical Exam. This exam must be done by a doctor who is approved by U.S. Citizenship and Immigration Services (USCIS). These doctors are called Civil Surgeons.

STEP 1: Schedule an appointment

For families: Please schedule a separate appointment for each family member, even if you are all coming at the same time. This ensures we can create individual patient charts and send each person their own forms.
STEP 2: Sign Up/Login To Elation Portal
Once your appointment is scheduled, you will receive an automated email with a link to complete the Patient One-Time Visit Consent Forms.
You will also receive an invitation from DPC Family Doctor to sign up for and access the Elation Health Passport Portal, our secure platform for confidential communication and access to your health records.
All future communication and document sharing will take place through the portal, so please make sure to sign up and log in.
STEP 3: DPC Family Doctor – I-693 Immigration Medical Exam Instructions
We appreciate your cooperation in logging into the Elation Health Passport Portal for secure and confidential communication of your personal and medical information.
Through the Elation Portal, please upload the following documents:
1) A photo ID or passport copy for our records
2) Any previous vaccination history

Kindly fill out the first two pages only (DO NOT SIGN THEM)
For the remaining pages, please print the NAME section at the top of all pages, starting from page 2 through page 14
